I have just answered by email:
A similar problem was reported by another customer in the past who printed from Illustrator with the page size setting "Custom" in the Illustrator print dialog. The media size setting in Illustrator should always be "Defined by driver" and you can select the page size you want with the button "Page Setup...".
If you need a custom page size, then
* In the Illustrator print dialog, select
"Media Size"="Defined by Driver".

* Open the "Page Setup..." dialog and select
"Format for"=<your printer model

* From the drop down list, select the page size
or select "Manage custom sizes" if you need to define a new page
size:

- Make sure to select "Margins" = <your printer model>
- Then add a new page size (button "+"), enter a name and enter the
size.
- Select the size that you created and exit the "Page Setup..."
dialog.
* Check if the preview in the Illustrator print dialog looks correct.
